This form should only be used when:

A credit has been applied to your account following a Sports Booking cancellation made by you, and the cancellation was within the notice period eligible for a refund as outlined in our Cancellation Policy.

You have cancelled a Sports Booking due to exceptional circumstances and wish to request a refund, as outlined in our Cancellation Policy.

You have cancelled your entire Sports Block Booking, and the cancellation was within the notice period eligible for a refund as outlined in our Cancellation Policy.
